           Do braces cause staining
          &#xD;
    &lt;/span&gt;&#xD;

           After braces are removed from patients, sometimes (rarely) staining can be seen in the outline of the brackets. Many people believe braces cause staining on the teeth. Some believe they even cause cavities. Why would anyone get braces if they cause staining? Do braces really cause staining?
          &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      &lt;span&gt;&#xD;
        
            ﻿
           &#xD;
      &lt;/span&gt;&#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      
           The simple answer is “no”. In fact, if you look closely at teeth stained during braces treatment, you will notice the stain only surrounds the bracket, but the part of the tooth that was glued to and directly under the bracket is perfectly unharmed and healthy. The bracket actually protects the enamel where it attaches to the tooth.
          &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      
           So where does this staining come from?
          &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;br/&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      
           The harsh truth is that staining or cavities during orthodontic treatment are caused not by braces, but by poor patient oral hygiene, primarily poor toothbrushing technique.
          &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;br/&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      
           Braces are a very good “plaque trap”. Plaque accumulates on and around braces quickly and easily, primarily because they are harder to clean. Unchecked plaque leads to staining and eventually cavities. With proper home care, braces cause neither staining nor cavities. With proper brushing technique, the plaque has nowhere to hide, and staining will not occur.
          &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;br/&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      
           What do I do if I see staining?
          &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      &lt;span&gt;&#xD;
        
            The #1 thing to do is improve brushing technique. The stains mostly disappear over the few months following bracket removal, though some can be persistent. There are a few products that orthodontists or dentists can use to help the process in severe cases. Most people ask if they can undergo tooth whitening to resolve staining. Unfortunately, tooth whitening is a generalized process, so while it may whiten the stain, it will also whiten the parts of the tooth that are not stained, leaving the stained appearance to the teeth.
           &#xD;
      &lt;/span&gt;&#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;br/&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      
           Additionally, and counterintuitively, the stains are actually often white, but whiter than the normal tooth and it looks out of place. Tooth whitening also does not address the true molecular cause of the staining. Because of this, I always recommend waiting at least a few months for the body to naturally recover and improve the staining on its own before commencing with tooth whitening.
          &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;br/&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      
           How can my orthodontist help prevent staining and cavities?
          &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      &lt;span&gt;&#xD;
        
            The primary cause of staining and cavities is improper brushing technique. The best thing orthodontists can do is properly teach brushing techniques specific to patients with braces. Once that is done, it is up to the patient to maintain good oral hygiene throughout treatment. It takes a little practice, and might add one minute to your daily routine, but it is very simple.
           &#xD;
      &lt;/span&gt;&#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;br/&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      
           There are also certain products used when braces are placed that can help prevent staining preemptively, and you should ask your orthodontist about what he does to help prevent staining.
          &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;br/&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      
           As a last resort, orthodontists sometimes will remove brackets if they see consistent poor oral hygiene, namely in the form of plaque accumulation or significantly inflamed gingiva (gums). It would be better to have teeth without cavities and significant staining than to have straight teeth with cavities and staining. Continuation of teeth straightening can be done at a later point when the patient is more committed to oral hygiene.

           First and foremost, we should clarify: Invisalign is a brand name, like Kleenex or Zip Lock. Invisalign treatment is a brand of Clear Aligner Treatment (CAT). So, is CAT the best option?
          &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;br/&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      
           Clear aligner therapy is an amazing treatment modality. It has actually been used since the 1980s before Invisalign came along. The technology behind clear aligners has advanced recently to become a major method of treatment for orthodontists. In the hands of a well-trained orthodontist, CAT can be as effective as braces, and sometimes even more effective. However, there are also cases that still are not recommended for CAT over braces.
          &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      &lt;br/&gt;&#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      
           If Invisalign is an option, why would anyone do braces?
          &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      &lt;br/&gt;&#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      
           CAT is truly an excellent option. They are clear, they are removable, and they are flexible. This means they are essentially invisible to the untrained eye, you can eat, brush and floss normally, and orthodontists have exceptional control over the force systems applied to your teeth.
          &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;br/&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      
           However, there are a few advantages braces have, too. First, braces are “fixed”. This means they are attached to your teeth. For people – usually but not always kids and teenagers – who lose things or are not confident they would be compliant enough to wear clear aligners 22 hours per day as instructed, braces are a better option. 
          &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;br/&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      
           Orthodontists sometimes prefer braces because there are certain types of tooth movements that are simply more effective and better accomplished with braces. Lastly, braces allow for more flair. They can be bling for your mouth, where you can choose from dozens of color ties, and even between metal, clear, and gold colored brackets. Awesome. Use them as a statement. You can make braces cool, and there is no shame in wearing them. You should never be embarrassed that you are trying to improve yourself – show it off!
          &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      &lt;span&gt;&#xD;
        
            ﻿
           &#xD;
      &lt;/span&gt;&#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;

           General dentists do Invisalign, too. Isn’t that the same?

    &lt;/span&gt;&#xD;

    &lt;span&gt;&#xD;
      
           It’s not the material or method of treatment that makes an orthodontist an orthodontist. It is the level of care delivered due to a specialized understanding of diagnosis and treatment planning and understanding the mechanics and biology of tooth movement. If you would not go to a general dentist for braces, you shouldn’t go for Invisalign.

           This is something I hear almost every week from patients. The wisdom teeth grow in (or try to) and push the back teeth forward until the bottom teeth have no room and become crowded. It seems to make sense. Interestingly, research studies have never shown such a causative relationship. So, what gives?
          &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      &lt;br/&gt;&#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      
           The bottom front teeth tend to get crowded most in the late teens and early 20s. Wisdom teeth tend to  also complete their development in the late teens and early 20s. This is why they are so commonly associated. However, research has shown that the same exact crowding occurs even in people who are congenitally missing their wisdom teeth. In other words, even when someone never grows wisdom teeth, the other teeth shift anyway.
          &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      &lt;br/&gt;&#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      
           OK, so why did your teeth all of a sudden get crowded?
          &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      &lt;br/&gt;&#xD;
    &lt;/span&gt;&#xD;

           Unfortunately, there is a natural shift of the lower back teeth, moving forward slowly over time. This is why you’ll also see this same issue come in people who have had their wisdom teeth removed or those in their 50s who have had wisdom teeth in their mouth for 30 years. Your teeth probably didn’t get crowded all of a sudden, but it has likely been a very long, drawn out process that finally reached a point that you started to notice it. This is also the reason why you have to wear retainers after orthodontic treatment into adulthood.

Well, I have had the pleasure (or displeasure?) of practicing in many different types of practices: Corporate, private equity backed practice, practices owned by orthodontists, practices owned by general dentists, a practice run by a lawyer and faculty practice in a dental school. As you can imagine, patient care varies wildly across these different practices. 
          &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      &lt;br/&gt;&#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      
           But, what I learned from all of them is that there is a need for better care, and I want to provide that to my patients. I was even once let go from a practice because I wasn’t willing to lower my standard of care! Their words, not mine. And I wear it as a badge of honor. I was not willing to sacrifice the quality of my work so that I could see more patients for the sake of the practice’s bottom line. If a patient starts treatment, they deserve the absolute best. They deserve my time and my expertise. 
          &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      &lt;br/&gt;&#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      
           What was also common in many of these practices was rapid turnover of the doctors treating the patients and sometimes the staff as well. Some patients I saw have seen four orthodontists! That is not a recipe for the highest level of success. Owning my own practice was the best way to improve on all of these aspects. We can make sure we maintain continuity of care to the highest standard using the most modern techniques. 
          &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      &lt;br/&gt;&#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      
           As the landscape of orthodontics changes, it is becoming less and less common to find a private orthodontic practice. We think that is to the detriment of the patient, and we want to bring the level of care you deserve back to you.
          &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      &lt;br/&gt;&#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      
           To reach our goal of owning our own practice, we had opportunities to purchase existing practices, which would have been much easier and less stressful than starting from scratch. But these were not our own, and they were not in our community. We wanted to create our own brand with our own vision. We believe our patients deserve the best of the best, and we wanted to develop and start a boutique, concierge practice in our community. If our patients are not happy, we are not happy.
            &#xD;
      &lt;br/&gt;&#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
